What each one is approved to treat
Ozempic is indicated in adults with type 2 diabetes as an adjunct to diet and exercise to improve blood sugar control, to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes and established cardiovascular disease, and to reduce the risk of sustained eGFR decline, end-stage kidney disease and cardiovascular death in adults with type 2 diabetes and chronic kidney disease. Weight management is not one of its labeled indications.
The Wegovy label covers several uses, each in combination with a reduced-calorie diet and increased physical activity where stated:
- To reduce the risk of major adverse cardiovascular events (cardiovascular death, non-fatal heart attack or non-fatal stroke) in adults with established cardiovascular disease and either obesity or overweight.
- To reduce excess body weight and maintain weight reduction long term in adults and children 12 and older with obesity, and in adults with overweight who have at least one weight-related condition.
- To treat noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) with moderate to advanced liver fibrosis in adults. The label says this indication was approved under accelerated approval and may depend on confirmation of clinical benefit in a further trial.
Because they share an ingredient, the Wegovy label says that using Wegovy with other semaglutide-containing products or with any other GLP-1 receptor agonist is not recommended.
Dose ranges and titration
Both start at 0.25 mg once weekly for 4 weeks and increase gradually to reduce the risk of digestive side effects. Where they diverge is the maintenance range.
| Weeks (Wegovy schedule) | Ozempic® (increase only if needed) | Wegovy® injection |
|---|---|---|
| Weeks 1 to 4 | 0.25 mg | 0.25 mg |
| Weeks 5 to 8 | 0.5 mg | 0.5 mg |
| Weeks 9 to 12 | 1 mg if more blood sugar control is needed, after at least 4 weeks on 0.5 mg | 1 mg |
| Weeks 13 to 16 | 2 mg if more control is needed, after at least 4 weeks on 1 mg (the maximum) | 1.7 mg |
| Week 17 onward | 0.5 mg, 1 mg or 2 mg weekly, based on blood sugar control | 1.7 mg or 2.4 mg (2.4 mg recommended); 7.2 mg is the maximum for patients who tolerate 2.4 mg for at least 4 weeks and need more weight reduction |
Ozempic increases are based on blood sugar control, so some patients stay at 0.5 mg or 1 mg. For patients with type 2 diabetes and chronic kidney disease, the Ozempic label sets 1 mg weekly as the maintenance dose after at least 4 weeks on 0.5 mg. If a Wegovy dose is not tolerated, the label says to consider delaying escalation for 4 weeks.
If two or more consecutive Wegovy doses are missed, the label says to restart escalation at a lower dose. See our full GLP-1 dosing schedule guide for side-by-side schedules.
Pens and presentations
Ozempic is supplied in single-patient-use pens that hold multiple weekly doses (2 mg/3 mL, 4 mg/3 mL and 8 mg/3 mL pens) and in single-dose prefilled syringes. Wegovy injection is supplied in single-dose pens, single-dose syringes and a WEGOVY® FlexTouch® single-patient-use pen (9.6 mg/3 mL) that delivers four 2.4 mg doses. A 7.2 mg/0.75 mL single-dose pen is also listed.
Both labels say to inject under the skin of the abdomen, thigh or upper arm once a week, at any time of day, with or without meals, and to rotate injection sites. The Wegovy label also says to train patients on the specific presentation they receive. Our guide on how to inject a GLP-1 medication covers technique.
Safety and side effects
Both labels carry a boxed warning about thyroid C-cell tumors seen in rodents. Both are contraindicated in people with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2, and in people with a serious allergic reaction to semaglutide. Warnings in both labels include pancreatitis, severe digestive reactions and hypersensitivity. The Ozempic label also lists diabetic retinopathy complications, which were more frequent with Ozempic in a trial of patients with type 2 diabetes and high cardiovascular risk. See our guide to GLP-1 side effects.
What the Wegovy trials found
Weight-loss evidence belongs to Wegovy, not to Ozempic or compounded products. In the Wegovy label's 68-week trial of 1,961 adults with obesity or overweight and at least one weight-related condition, average weight change was a 14.9% loss with Wegovy 2.4 mg vs 2.4% with placebo, alongside diet and activity counseling. In the label's cardiovascular trial of 17,604 adults with established cardiovascular disease and obesity or overweight (without diabetes), Wegovy reduced the risk of a first major cardiovascular event, with a hazard ratio of 0.80 (95% CI 0.72 to 0.90) compared with placebo. These are averages for the approved brand-name drug in study populations, not predictions for any one person.

Frequently asked questions
Are Ozempic and Wegovy the same thing?
They contain the same active ingredient, semaglutide, and both are once-weekly injections from Novo Nordisk. They are separate FDA-approved products with different labels, approved uses and dose ranges.
Is Ozempic approved for weight loss?
No. Ozempic is approved for adults with type 2 diabetes, to improve blood sugar control and to lower certain cardiovascular and kidney risks in that group. Wegovy is the semaglutide injection approved for chronic weight management.
What are the doses of Ozempic and Wegovy?
Both start at 0.25 mg once weekly for 4 weeks. Ozempic maintenance doses are 0.5 mg, 1 mg or 2 mg weekly. Wegovy maintenance doses are 1.7 mg or 2.4 mg weekly, with 2.4 mg recommended, and up to 7.2 mg for some patients who tolerate 2.4 mg and need further weight reduction.
Can I take Ozempic and Wegovy together?
No. The Wegovy label says using it with other semaglutide-containing products or any other GLP-1 receptor agonist is not recommended. A prescriber chooses one product at one dose.
Does Gala offer Ozempic?
Gala offers brand-name Ozempic, prescribed when clinically appropriate, along with compounded semaglutide and compounded tirzepatide. Compounded medications are not FDA-approved. A licensed provider reviews your online assessment and prescribes only if it is appropriate for you.
